We understand that as a student it is very critical that you are mentored and trained by the very best to develop your skills and ignite your passion for healthcare.
Required Documents:
- Signed Affiliation Agreement if not already on file
- Compliance Letter (Clinical Students only)
- See Education Downloads below. Return answer sheet and signed forms.
Clearance: Once packet is received, it will be reviewed and the school will be notified whether the student is cleared to begin clinical rotation.
