Before You Arrive
To ensure that your admission process goes smoothly, there are several important items you should bring to the hospital. These items vary depending on the service area you will be visiting so please review the lists below.
Most outpatient visits, including lab, x-ray and some oncology procedures, require you to register in Admissions. Always check in at the registration desk so the staff can process your registration or direct you to the appropriate service area if registration is not required.
Emergency Department Visit
Please bring the following items:
- Insurance Card · Identification · Social Security Number
- List of medicines, herbs and supplements currently being taken
- Be sure the list of medicines includes dosages & times taken
- Bring medicines if possible
Outpatient Visit
Please bring the following items:
- Insurance Card · Identification · Social Security Number
- Doctor’s Order · Pre-certification numbers for MRI and CT scans
- Referrals (if needed) from primary care physician
Inpatient Visit
Please bring the following items:
- Insurance Card · Identification · Social Security Number
- Any papers your doctor gives you
- List of medicines, herbs and supplements currently being taken
To start your registration process, come to our registration areas located inside the Main Entrance and at the Emergency Department Entrance (on the corner of Rockford and Worth Streets). Parking is available at both entrances and in the large parking area across from the Hospital on Worth Street.
Our registration clerks will guide you through this important process quickly and easily as possible. Please remember to bring the items listed above to ensure that we have all of the information that is required.
