My Northern Chart Patient Portal
You may access much of your current patient information regarding radiology and lab results, mammography reports, hospital reports, inpatient stays, and Emergency Department visits through our private, secure, online patient web portal.
To access your personal patient portal you must request an access code during your hospital visit or request access at My Northern Chart, or contact a Health Information Representative at 336-719-7300. A valid e-mail is required to register. Internet Explorer cannot be used as the web browser.

Medical Records Office
Printed copies of your medical records/health information may be requested from the Health Information Management Office located on the 1st Floor of the Hospital. It is recommended you call 336-719-7006 to make an appointment to pick up your medical records. Please allow 72 hours advance notice and plan to bring a picture ID for verification and security purposes. A completed Release of Information Authorization will be required to obtain medical records. All guidelines related to the Health Information Portability and Accountability Act (HIPAA) will be adhered to.
Copies of your records requested for Continuation of Care may be sent directly to your provider at no cost to you. If you are requesting printed pages from your medical record for personal use, printed copy charges will apply.
Charges for printed copies are as follows:
- Pages 1-25 are 75 cents per pages
- Pages 26-50 are 50 cents per page
- Pages 51+ are 25 cents per page
